Effective as of the date shown below, The Party/Guardian of Party “Undersigned”, understands and grants Hosanna Christian Fellowship, the use any photos, videos, live streaming, recordings, and/or voice of any persons identified below. The “Undersigned” also acknowledges that the use of any media (past, present, and future), is covered under this consent. 
            The “Undersigned” represents, understands, and warrants that:
  • No other party has been granted an exclusive license with respect to the Image and/or voice,
  • No other party’s authorization or consent is required with respect of the permission granted to the Released Party under this Consent and Release,
  • No promises or monetary payments have been made in exchange for signatures,
  • Signee is of legal age and has the right to sign this form for any underage participant.
            Any Image, Video, and/or Voice may be copyrighted, used and/or published individually or in conjunction with other photography, video works, and recordings; and could be use in any medium (including but not limited to, printed publications, public broadcast, digital format) or for any lawful purpose including but not limited to; trade, exhibition, illustration, promotion, publicity, advertising, and electronic publication.
            THE UNDERSIGNED WARRANTS THAT THE “PARTY/GUARDIAN OF PARTY” HAS READ AND FULLY UNDERSTANDS THIS CONSENT. BY SIGNING, THE “UNDERSIGNED” HEREBY RELEASES ANY LIABILITY FROM HOSANNA CHRISTIAN FELLOWSHIP AND FREELY ENTERS INTO THIS CONSENT AND SUBSEQUENT RELEASE OF ANY MEDIA.
